JUSTIFICATION OF ABSENCE/MAKE-UP

FOR INFORMATIONAL PURPOSES ONLY! THERE IS NO LONGER A FORM TO FILL OUT. Speak with Prof. Flores regarding make-ups, attendance adjustments, etc. The following information is provided so that students can have a general idea of what is and is not considered excused or unexcused.

INFORMATION AND REQUIREMENTS FOR MAKE-UPS & ATTENDACE ADJUSTMENTS

Excused absences include, but are not limited to:

a) observance of religious holy days, b) required court appearances, c) naturalization and oath ceremonies, d) military service, e) emergency/health problems of student or student’s family that require treatment, hospitalization, or other types of care not allowing a student to be in class, f) official school business such as team meeting, performances (where student is a performer!), and other events where the student’s presence and participation is required for the success of the event or by terms of agreement.

Acceptable documentation: a letter from a court magistrate requiring your appearance on the day and time of appearance; a doctor’s note excusing you on the day and time of the assignment; a letter from a coach/director of an official HCC event explaining your absence on the day and time of the assignment; a towing receipt with location and time for your broken down car; a letter/flyer from a funeral home; and so on. (NOTE: HIPAA guidelines protect the privacy of you and other’s medical information. I am not asking for specifics of illness or other personal information. I am asking that your medical professional provide a note saying that you could not complete or turn in an assignment as a result of a Doctor’s directive.) Documentation must have appropriate contact information (business name, contact name, telephone number, business address, etc.) so I can call and verify. All documentation needs to be from an official, not a parent, sibling, friend, etc. NO DOCUMENTATION? NO MAKE-UP. NO EXCEPTIONS. CONSIDERATION OF MAKE-UPS AND ATTENDANCE ADJUSTMENTS WILL REQUIRE APPROPRIATE DOCUMENTATION. YOU MUST SPEAK WITH PROF. FLORES BEFORE HAND.

Unexcused absences include, but are not limited to:

Oversleeping/alarm failure, needing sleep or rest, missing the bus/ride, car trouble, needed at home/babysitting, employment/job interview, having to be at work, family business overseas, shopping/errands, personal grooming appointments (hair, nails, tanning, etc.), driver's education, vacations, traffic, personal time, "submission button not working," running out of time, etc.., or any other reason that I have deemed unacceptable. You have a job to do: come to class (F2F) or log-on (on-line) and do your work. It is up to you to see to it that this happens.
