Folder Supplier-Vendor Number Request Paperwork (For Jurors and Artists) DO THIS PAPERWORK WELL BEFORE YOUR WORK IS PERFORMED SO YOU CAN GET PAID. PLEASE SAVE COPIES AND CORRESPONDENCE. If you are a registered HCC vendor (and have a scheduled arrangement to be a juror or exhibitor) you can: -receive a $100 honorarium if you judge a student exhibition -receive a $100 dollar honorarium if you have an "Artist's Talk" in the gallery during your show reception or special event/class. Once you are an established HCC vendor, getting paid is easy. If you are not established yet, the process takes 2 - 6 months. This process is a bit trickier if international paperwork is involved. We have 20 student shows per year at 5 galleries across the district, so once you are on the registered vendor list, you will probably get contacted multiple times per year to judge student shows. Here is the process: 1. Print this form. 2. Complete it and attach a completed W9 form. 3. Scan the forms and email them to [email protected]. 4.You might want to add a few lines to Jeremy about which show or dates this will be for. Jeremy Tidwell will complete Section 2 and send it on to the Procurement staff. 5. copy the completed form/files to me. That way I can follow-up on your behalf and shepherd the process along. You may be puzzled by "Supplier Diversity" and "Professional Services Provider" etc. (Those are for other vendor businesses and services.) Those don't apply to jurors, but HCC procurement still wants us to use this form. Just fill in all the boxes that apply and put an "?" in any box that doesn't seem to apply. Jeremy and I will follow up as necessary. File Blank Juror Form This is the form that jurors will be asked to sign when they come to judge the shows. This sample form is blank. The details will be entered by the HCC Art department prior to signing.
